What we collect
Your first and last name, email address, professional role, questionnaire answers, the time taken on each statement, and an irreversible digest of your IP address used only to limit abusive attempts.
Why
To produce and send you your report; to let you retake the questionnaire and compare profiles over time; to improve the instrument through statistical analysis on aggregated data. Your answers are never sold, rented or shared for advertising.
If your employer gave you the code
The organization that issued your access code can see your profile and the corresponding team reading. It does not have access to your statement-by-statement answers. If this was not disclosed to you beforehand, write to us: you may ask to be removed from the team reading.
Where and for how long
Information is hosted in Canada. It is destroyed or anonymized no later than thirty-six months after the assessment. Once anonymized, answers continue to feed instrument statistics without any possibility of linking them to you.
Your rights
At any time you may request access to your information, its correction, its deletion, or the withdrawal of your consent. Write to the responsible person; you will receive a reply within thirty days. If it does not satisfy you, you may file a complaint with the Commission d’accès à l’information du Québec.
